Please read this Telehealth Informed Consent carefully before you schedule or attend a visit with Fitnera Clinic. By booking an appointment, creating a patient account, or taking part in a telehealth visit, you confirm that you have read, understood, and agreed to this consent.
1. What Telehealth Is
Fitnera Clinic ("Fitnera," "we," "us," "our") is a telemedicine practice. We do not see patients in person. Your care is delivered through telehealth. This means we use secure electronic tools to evaluate, diagnose, treat, and advise you. These tools may include live video, audio (telephone), secure messaging, electronic forms, photographs you upload, and review of your records and test results.
Our telehealth services are available only to adults 18 years of age or older who are physically located in Missouri or Illinois at the time of each visit.

3. Limitations and Risks of Telehealth
By using Fitnera's telehealth services, you understand that:
Physical examination is limited. Your clinician cannot touch, listen to, or directly examine you the way they could in person. Some conditions cannot be safely diagnosed or treated by telehealth.
You may be referred elsewhere. Your clinician may decide that telehealth is not appropriate for your concern. If so, you may be told to seek in-person care, urgent care, laboratory testing, imaging, a specialist, or emergency care. Fitnera does not perform procedures, surgery, or in-person examinations.
Information may be incomplete. Poor video or audio quality, or incomplete information, may affect the accuracy of your evaluation. You agree to provide complete and truthful information about your health history, medications, allergies, and symptoms.
Technology can fail. Connections may be interrupted or delayed. If a video visit fails, your clinician may continue by telephone or reschedule the visit.
Privacy risks exist. Fitnera uses reasonable, HIPAA-compliant safeguards. However, no electronic system is completely secure, and there is a small risk that information could be intercepted or accessed without authorization.
4. Emergencies
Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency department. Examples include chest pain, trouble breathing, stroke symptoms, severe bleeding, severe allergic reaction, or thoughts of harming yourself or others. For a mental health crisis, you may also call or text 988.
Messages sent through the patient portal, email, contact forms, or voicemail are not monitored continuously and must not be used for urgent or emergency concerns.
5. Identity and Location
At each visit, you agree to:
Confirm your identity. This may include photo identification.
Tell your clinician your physical location (city and state) at the start of the visit.
Be physically present in Missouri or Illinois during the visit. The visit may be stopped or rescheduled if you are located outside these states.
Provide a phone number where you can be reached if the connection is lost.
6. Your Responsibilities
Take part from a private, safe location, and not while driving.
Use a device with a working camera, microphone, and internet connection when a video visit is scheduled.
Follow your treatment plan and complete any recommended labs, follow-up visits, or referrals.
Tell Fitnera about any change in your health, medications, pregnancy status, or contact information.
Do not record any visit without the written permission of your clinician.
7. Prescriptions
Your clinician may prescribe medications when clinically appropriate. Prescribing decisions are made by your clinician using independent medical judgment. Requesting a medication does not guarantee it will be prescribed. Fitnera does not prescribe controlled substances unless permitted by law and determined to be clinically appropriate. Some medications require lab monitoring or follow-up visits before they are continued or refilled.
Medical weight-management services are also governed by Fitnera's separate Medical Weight Management Consent and Program Terms.
8. Records, Privacy, and Sharing Information
Your telehealth visits will be documented in your medical record, the same way an in-person visit would be.
Fitnera protects your health information as described in our Notice of Privacy Practices.
You authorize Fitnera to share relevant health information with pharmacies, laboratories, and other clinicians involved in your care, as permitted by law.
Fitnera does not record video visits unless you have been told in advance and have given permission.
9. Communication
You agree that Fitnera may contact you by secure patient portal, telephone, text message, or email about appointments, results, and care. Standard email and text messages may not be fully secure. You may ask Fitnera to limit how we contact you.
10. Fees and Payment
Telehealth visits are subject to Fitnera's current fees and payment policies. These are provided to you before scheduling or at the time of scheduling.
11. Your Rights
Your consent to telehealth is voluntary.
You may refuse or withdraw consent at any time by notifying Fitnera in writing at contact@fitnera.com or by mail to the address below. Withdrawing consent will not affect care you have already received. If you withdraw, Fitnera can help you find an in-person clinician.
You may ask questions about telehealth at any time, and you have the right to have those questions answered.
12. Duration and Updates
This consent applies to all telehealth services you receive from Fitnera Clinic. It remains in effect until you withdraw it in writing. Fitnera may update this consent from time to time. The current version will be posted on this page with its effective date, and you may be asked to review and accept the updated version before your next visit.
13. Your Consent
By booking an appointment, creating a patient account, or taking part in a telehealth visit with Fitnera Clinic, you confirm that:
You have read and understand this Telehealth Informed Consent.
You have had the opportunity to ask questions, and your questions have been answered.
You are 18 years of age or older.
You consent to receive health care services from Fitnera Clinic through telehealth.
Your clinician will also confirm your consent at the start of your first visit.
